incredible Skilled Nursing Facilities in KY is looking for a
superstar *Wound Care Nurse Practitioner* to be part of a
facilities-based team of medical providers who provide medical
services to patients living in long-term care facilities. While an
interdisciplinary care team to provide medical care to patients in

Qualifications:** Current, unrestricted NP or PA license in the
state of KS.* Board Certification.* DEA licensure.* Clinical
background in wound care.* Experience working in a long-term care
setting.* Ability to work with an EMR.* Ability to manage patients
with advanced illness and multiple chronic conditions.* Ability to
work within an interdisciplinary care team to coordinate patient
care.* Wound care experienceThe job description of a *Wound Care
Nurse Practitioner* includes conducting physical examinations,
Provide wound care for long-term care patients residing in skilled
nursing facilities. As a Nurse Practitioner, you'll have to work
closely with other healthcare professionals, so you should have a
team spirit and a positive attitude. Patience, problem-solving
skills, and communication ability are important.*Responsibilities
include but are not limited to:** Prescribe appropriate medications
.* Order, review, and/or follow-up with treatments and diagnostic
testing.* Communicate with the nursing home staff on a regular and
as needed to discuss changes in condition. and/or the care plan.*
Collaborate with primary care physician.* Provider will assess and
monitor residents on a regular basis.* Provider will be available
to assist staff in assessing wound and pressure ulcer risks.*
Provider will evaluate multiple factors related to wounds: i.e.*
Nutrition, Mobility, Incontinence associated dermatitis, Pressure
ulcers, Diabetic ulcers, Arterial ulcers, Venous ulcers, and Common
dermatologic conditions*MANAGEMENT** Measures to decrease the risk
of wounds with recommendations to staff.* Measures to optimize
healing via protocols* Superficial wound debridement* Dressing
selection and management* Placement of negative pressure wound
dressings* Post myocutaneous flap surgery care*#NP1*Job Type:
