Job Description:

Implements, coordinates and evaluates all clinical Pharmacy services, including JCAHO standards. Assists and directs MUEs (Medication Usage Evaluations), collaborates with the medical staff, and establishes criteria for the safe, effective and appropriate use of medication. SPECIFIC . Will provide services to patients from birth to death. Essential Duties and Responsibilities include the following. Other duties may be assigned.

• Manage a team of pharmacists and pharmacy technicians in an ambulatory clinic setting. Ensure that all direct reports are fully trained to operate at the level dictated by the needs of the patients and the providers. Responsible for financial and quality metrics for the ambulatory pharmacy clinic team.
• Review records of patients to determine the appropriateness of medication therapy, and to identify patients for program outreach. Evaluate patient’s condition(s) and any barriers to medication compliance to ensure all issues are being treated. Develop effective medication plans that minimize the risk of adverse side-effects. Identify untreated health problems and refer patients to appropriate physicians or other clinical staff. Collaborate with healthcare professionals to ensure optimal patient care.
• Provide medical staff with appropriate medication information for new and existing drug therapies to support safe prescribing practices.
• Responsible for developing and overseeing ambulatory formulary management, smart pathways and protocol development. Additionally, responsible for the development of procedures, medication evaluation and the standardization of ambulatory care. Coordinates, and evaluates area-wide programs and projects as assigned. Also, required to attend regular pharmacy meetings and committee meetings as assigned by leadership.
