Job Description:
Midland Care's Topeka PACE Center is now hiring for Licensed
Master Social Workers!
Summary
We are seeking a Licensed Master Social Worker (LMSW) to join our
team at our Topeka PACE Center. As an LMSW, you will be responsible
for providing comprehensive social work services to the elderly
population in the PACE program. You will work closely with a
multidisciplinary team to assess and address the psychosocial needs
of participants and their families, as well as coordinate and
facilitate care planning and supportive services.
Midland Care's Program of All-inclusive Care for the Elderly (PACE)
is a not-for-profit health care program that helps people at risk
for needing nursing home care who meet a level of care requirement
stay in their own homes and communities.
For Midland Care PACE participants, PACE means a team of
experienced doctors, nurses and other health care professionals
getting to know the participant and working with him or her to
design a plan to aid in achieving health goals.
Employment Status: Full-Time-Monday - Friday, 8:00 a.m. to 5:00
p.m.
Pay range beginning at $54,000/year - pay based on experience
$3,000 Sign-On Bonus

Qualifications
Required: Current unrestricted State of Kansas license to practice
as a Licensed Master Social Worker. Must have knowledge and skills
with regard to health care coordination, pain/symptom management,
crisis intervention and counseling. Must possess valid Kansas
Driver's License, in force automobile liability insurance and
maintain a Motor Vehicle Record (MVR) that meets organization's
insurability standard as defined by Midland policy.
