together. /n The Foster Care, Care Coordinator is responsible for
using trauma-informed evidence-based approaches with members and
caregivers for children and youth who are in out of home placement
through the foster care system with complex/high or moderate needs
who are not enrolled in a home and community-based waiver, on a
wait list or receiving services through a Certified Community
Behavioral Health Clinic (CCBHC). /n This is a field-based role
located in Sedgewick, Harvey, Butler or Sumner County, KS and
surrounding areas. /n If you reside within Sedgewick, Harvey,
Butler or Sumner County, KS, you will enjoy the flexibility to
telecommute* as you take on some tough challenges. /n Primary
Responsibilities: /n /n - Assess, plan and implement care
strategies that are individualized for children and youth in a
family-oriented context (e.g., considering reunification and/or
other permanency planning needs, needs for adoptive children, and
needs for transition-age youth with alternative permanency plans)
and directed toward the most appropriate, lease restrictive level
of care /n - Identify, initiate, coordinate, and monitor referrals
for psychosocial, behavioral and medical services /n - Manage the
care plan/plan of service throughout the continuum of care as a
single point of contact /n - Communicate with all stakeholders the
required health-related information to ensure quality coordinated
care and services are provided expeditiously to all members /n -
Advocate for children, youth, and families as needed to ensure the
member’s needs and choices are fully represented and supported by
the health care team /n - Provide trauma-informed and
disease/disorder specific education to members and caregivers /n -
Coordinate with the child welfare and juvenile justice system on
behalf of members and caregivers /n - Coordinate with additional
services, including but not limited to, educational/educational
advocacy, complex behavioral health (PRTF, etc.), services in line
with state Family First Prevention and Services Act for children
and families, high fidelity wraparound services, early childhood
intervention /n - Complex cases will have additional oversight by
master’s level case manager, including review of assessments and

you may be interested in. Required Qualifications: /n /n - Must
have one of the following: /n /n - Master’s Degree in Social Work,
Nursing, Psychology or related human services area /n -
Unrestricted, independent licensure as a Registered Nurse /n -
Bachelor’s Degree (in Social Work, Rehabilitation, Nursing,
Psychology, Special Education, Gerontology, or related human
services area) /n /n - 2 years of experience working within the
community health setting in a healthcare role /n - 1 years of
experience working with multi-system children /n - 1 years of
experience working with MS Word, Excel and Outlook /n - Ability to
travel in assigned regions to visit Medicaid members in foster
homes and/or other settings, including foster care agencies,
community centers, hospitals, or providers' offices /n - Access to
reliable transportation & valid US driver's license /n /n Preferred
Qualifications: /n /n - Licensed Social Worker or clinical degree
/n - Experience with electronic charting /n - Experience with
arranging community resources /n - Background in managing
populations with complex medical or behavioral needs /n /n *All

salary range for this role is $28.61 to $56.06 per hour based on
full-time employment. Pay is based on several factors including but
not limited to local labor markets, education, work experience,
certifications, etc. UnitedHealth Group complies with all minimum
