Get to Know Your Team: The University of Kansas Health System St.
Francis Campus, a part of a rich legacy of compassionate care since
1909, now offers 378 licensed beds, a history of innovation, and
strong community support. Responsibilities Provide direct discharge
planning services and assistance with psychosocial services for the
patient and family. Collaborate with physicians and other members
of the healthcare team to develop a discharge plan in relation to
the patients health care needs. Utilize broad experience and
knowledge of appropriate healthcare resources for patients of all
ages. Promote continuity through communication of the patients plan
of care. Report to the Clinical Resource Management Coordinator.
Qualifications Job Requirements Master of Social Work (MSW)
Clinical emphasis from a CSWE accredited graduate school Kansas
licensure at the LMSW level BLS certification must be obtained
within 14 days of hire or transfer into the role and prior to
providing direct patient care. Preferred Job Requirements Two
semesters of graduate field experience in healthcare setting for a
new graduate or previous experience in a healthcare setting for an
